Consent and Release for Transportation

Participants, parents or guardians of participants in the Boothbay Sea and Science Center (BSSC) programs agree to this policy when registering for any program online or in person. This release is for your, understanding, executing, and signature online on your Active Network Account.

Parents, participants or guardians of participants hereby consent to participation in the 2023 summer program which includes daily travel from and to Boothbay Harbor, Maine to Boothbay Sea And Science Center location in Boothbay, Maine by transportation provided by BSSC, at an assigned offsite “pick-up” and “drop-off” time and place. *

In consideration of BSSC requiring the participants, guardians of child or children participating in the program to travel during the  summer program by transportation provided by BSSC,  I/We unconditionally release, hold harmless, waive, discharge and covenant not to sue the Boothbay Sea and Science Center, its directors, officers, employees, coaches, volunteers, and agents from any and all liability for damages and costs and any claim or demand therefore, directly or indirectly arising out of or related to any injuries or damage that the above named participant, person, child or children might sustain while engaged in the  travel or the program.

The participants, parents or guardians understand and acknowledge that participation in all programs as well as travel to  special events  and places can involve risks which include, but are not limited to, bodily injury, partial or total disability.   The social and economic losses or damages which can result from these risks and dangers can be severe and that not all risks may be known or reasonable foreseeable. I accept responsibility for losses or damages resulting from all such risks and dangers involved in the participation in all activities of The Boothbay Sea and Science Center

I agree to take appropriate precautions for my own safety and that of others. I will advise the persons in charge of any unsafe conditions or behaviors of other  participants.

*Assignment will be emailed before child’s first day in their program.
